Should exercise be painful?

When assisting a patient with rehabilitation from injury or pain, a major question that needs to be asked is "should this patient exercise through pain or not?" This becomes even more pertinent if the patient's condition consists of primary pain (1). And there have certainly been arguments for both sides on this topic (2-4) as pain management has been emerging as an increasingly relevant matter in current medicine.

Most practitioners realise that that there cannot be a "blanket rule" that can be applied to all, but every patient is an opportunity for a practitioner to clinically apply biopsychosocial framework followed by a construction of case formulation (5, 6). Thus the decision to exercise through pain or not is usually a destination that a practitioner can naturally arrive after such preceding thought-processes.

There are four important factors to consider when deciding whether or not a patient should exercise “through pain”:

 

1) Pathophysiology of the current condition
2) Mechanism of the pain being experienced
3) Patient’s attitude and beliefs
4) Level of endogenous modulation

 

1) Pathophysiology of the current condition

Perhaps the most obvious one; certain pathophysiological states logically indicate that exercising in pain would not be a sensible choice: e.g. acute inflammation post traumatic injury with tissue disruption. However some conditions are not as clear-cut: e.g. chronic patellofemoral pain. In such instances critical analysis of the diagnosis would be necessary before a decision is made: if a clinician was to reason that such patellofemoral condition was a result of muscle dysfunction from pain inhibition (7), it may be more reasonable to exercise without pain as further pain is likely to reinforce inhibition, especially in the early stages.

2) Mechanism of the pain being experienced

"Mechanism-specific approach" classifies pain into four categories (8, 9): nociceptive, inflammatory, neuropathic and dysfunctional (or nociplastic). While there are no “hard-guidelines” that can clearly define a given pain into such categories, it is not difficult to make a hypothetical call after a thorough case formulation construction, and such approach is absolutely necessary to provide a foundation of care for complex multi-dimensional conditions such as primary pain. Once mechanism of pain is hypothetically determined, the implication of experiencing such pain while exercising becomes clearer; e.g. certain nociceptive pain may be ignored, or even beneficial, whereas inflammatory and neuropathic pain is generally best to be avoided. Dysfunctional / nociplastic pain will need to be deconstructed - i.e. cause of the pain analysed - before determining the possible effects of experiencing such pain while exercising.

3) Attitude and beliefs

Pain excitation or inhibition from cortical and subcortical centres are well-documented through literature (10-12) and anecdotes, and the main constructs behind this powerful process are the attitude and beliefs of the patient. This is where fear-avoidance and catastrophising exist at one extreme, and placebo response and attentional modulation at the other. How a clinician positively “manipulates” these is critical to the success of the rehabilitation and is often dependent upon the “art” of communication and persuasion skills. Graded exposure (13, 14) is one of the most important and widely-used cognitive framework on which exercise therapy can be effectively integrated.

4) Suspected level of endogenous modulation

This resides closely to “attitudes and beliefs” as their end-effect is also endogenous modulation. However there are many other factors that can influence endogenous modulation (e.g. genetic, DNIC, autonomic regulation, gender, age, fitness, physical and psychological history) and these are important data that need to be considered to hypothetically assess the “activity level” of patients' endogenous pain modulation. In essence, stronger the modulation is suspected, stronger the exercise stimulation may be, and possibility of exercising through pain.

It is critical to specify the intention or aim behind exercise programmes when prescribing to patients as this will often give a quick indication of how the experience of pain should be handled. For example, in a case of mobilisation of joints and soft tissues post fracture with no significant pain issues, it is probably useful to “stretch into the pain zone" if this coincides with the restriction in range of motion and evidence of tissue shortening (i.e. nociceptive pain).

Compared to this in management of primary pain the goals are, in most cases, to modulate pain and facilitate neural recruitment. Thus how a clinician teaches patients to handle pain when it inevitably occurs during exercises - whether to alter exercises to minimise pain or to "ignore and keep going" - will need to be carefully considered in accordance with abovementioned factors to ensure the best outcome.

Fibromyalgia - part 2: the context of the diagnosis

In order to recover from fibromyalgia, there are two things that have to be done:

1. Stop doing things that are stuffing you up

2. And instead do things that help you.

First of all let’s look at the things that I have seen so many people with fibromyalgia do, that actually perpetuate the condition rather than assisting in recovery.

I will go through four most common and critical mismanagement of fibromyalgia. You could say they often occur somewhat in this sequence and they are VERY closely related.

 

1. Lack of appreciation of the context involving the diagnostic term “fibromyalgia”

2. Misinformation on the condition

3. Over-identification or emotional investment on the condition

4. Mal-adaptation that follows on the body

 

The first important thing is that these are not easily identifiable due to the fact that they are “below the surface”. But you sure feel the effect.

The mismanagement that I would like to discuss in this video is the first one: lack of appreciation of the context involving the diagnostic term – “fibromyalgia”.

First of all, let’s define fibromyalgia (and I’d like to add that most “fibromyalgia” patients that I see are often not even aware of this to begin with, which pretty much underlines the point that I want to raise):

 

Fibromyalgia is diagnosed in patients with chronic widespread pain and associated symptoms of fatigue, unrefreshing sleep, or cognitive dysfunction (or a combination thereof) for at least 3 months.(1)

The key here is that the symptoms that pertain to the diagnostic criteria (pain, fatigue etc) sit within a continuous spectrum; some are very mild and borderline fibromyalgia while others can be full-blown and significantly debilitating. And not only that, even though it is generally accepted nowadays that the symptoms of fibromyalgia are the result of dysfunctions in endogenous pain modulation(3-5), it is very difficult to establish a measurable relationship between the dysfunction and the symptoms experienced. 

All this means is that when a diagnosis of fibromyalgia is given, it does not (and should not) mean anything more than the exact definition given above; nothing more, nothing less – i.e. fibromyalgia is simply a description of symptoms and has always been.

The problem occurs when the terminology “fibromyalgia” is associated with certain ideas or beliefs – ideas that may be true for the very severest of the spectrum (and that’s not many) – but everyone who receives the diagnosis is lumped together under that idea.

In my opinion and experience vast majority of patients who have been diagnosed as fibromyalgia could have easily had their symptoms explained as:

 

"a severe but temporary dysfunction of the body due to a series of events that overloaded the system, and/or non-events where there should have been for the benefit of the body, in combination with failure of the body to optimally adapt to the stimulus related to those events and/or non-events.”

 

An example of such event that overloaded the system may be a prolonged stress from work that may have gradually impacted on endogenous pain modulation. An example of non-event where there should’ve been, could be not knowing how to activate endogenous pain modulation when a body is genetically predisposed to pain facilitation.

Combined together, these things FACILITATE pain – and tip the balance towards pain facilitation away from pain modulation. Which means that you’ll be susceptible to feeling an unnecessarily amplified amount of pain regardless of the visible condition of the body.

I suggest that you take a moment to examine this diagram. If there are some aspects that you do not fully understand, please leave a comment – we’ll do our best to answer it.

Anyhow when the symptoms are explained in this way (that was just an example), as opposed to sticking a label such as fibromyalgia, patients can be free from the "baggage" that comes with the label; instead they can focus on finding insights necessary for an effective management for his or her own condition rather than ones that belong to the severest of all fibromyalgia spectrum (which unfortunately are the easiest to discover from terribly unreliable sources such as the internet). 

But regrettably, the culture of medicine and patients’ expectations hinge on "having a diagnosis" - neither clinicians nor patients are often satisfied without giving a condition a label. In fact, numerous clinicians and patients are more satisfied with having a diagnostic label that only explains the fact that certain symptoms exist, rather than giving and receiving an explanation consisting of reasonable and rather helpful hypotheses behind such symptoms;

Some patients will even tend to complain “oh the doctor/physio/specialist couldn’t come up with the diagnosis for my condition” when actually more scientifically accurate and thorough explanations are given.

So if you’ve been told that you have fibromyalgia. I encourage you to jettison the diagnosis label. While there is an innate urge to label symptoms, it really does not do you any favours. Going without diagnosis feels really weird. But trust me, it will make you lighter and free up your headspace so you can actually focus on more important things to get yourself better.

Over the next series of videos I would like to share a few things about fibromyalgia, or specifically, common mistakes in regards to the management of fibromyalgia both by health professionals and patients. 

The goal of these videos is to help you to see some different perspectives in regards to fibromyalgia which is a multi-dimensional conundrum, and even discover certain notions, beliefs or behaviours that might have been contributing to the perpetuation of the condition. Such discovery can be absolutely crucial for a successful recovery.

 

Because fibromyalgia is NOT like your normal “injury”; if you’ve *sprained a joint or broken a bone what do you do? You rest while the injury heals. If you’ve got a bad case of flu or other illness, what do you do? You rest and recover.

But it is a mistake to categorise fibromyalgia like that – in most cases if you sat on your backside and waited for fibromyalgia to improve, it won’t; in fact it will get worse.

You might be thinking that the reason for the persisting nature of fibromyalgia, the fact that the pain just seems to hang around and doesn’t leave you alone, is simply because you have fibromyalgia (which sounds pretty obvious).

However I would like you to think slightly differently: that the most common reason for the persistence of symptoms with fibromyalgia is not the condition itself but mismanagement of the condition.

What I mean by mismanagement is how the condition is HANDLED by both health professionals and patients. And it is such poor handlings that directly result in the active perpetuation of the condition.

In other words, and I’d like to say this with the kindest intention, what stops the successful resolution of fibromyalgia is, you. It all comes down to how YOU handle or mishandle the condition.

And whether you are aware of it or not there are MANY ways one can develop the very subtle and not-so-useful art of mismanagement.

 

So, over the next few videos, I would like to discuss most common and significant types of mismanagement that occurs to variable extents for ALMOST ALL fibromyalgia cases.

But for now, the most important thing is that you have to be OPEN to possibilities. If you are not, I hope that that viewpoint can be changed in near future with my help. Because yes fibromyalgia is complicated but WE have made it a lot more difficult than it should be, and turned it into a bit of a monster.

The most common mistake in rehabilitation

The most common mistake in injury rehabilitation and pain management

On the previous video we talked about the three principles of recovery from injury or pain. These were:

1. Settling the acuteness
2. Letting structures heal (only if there’s a relevant structural compromise)
3. Get on with your life

In context of chronic or persisting pain, the principle that gets mishandled most frequently is by far the last one - getting on with your life. (And there are a number of crucial reasons behind that that we won't get into in this video)

And when this action of getting on with your life is delayed unnecessarily, MALADAPTIVE changes occur with your body. And we're talking about changes that occur at all levels of the body (1,2) - muscular, hormonal, neural, behavioural, cognitive, etc.

And these maladaptive changes are often the main drivers of the chronic, persisting pain, NOT your degeneration or arthritis, NOT the difference of your leg length, and certainly NOT because your spine is "out".

Today I would like to go through a typical example, a patient of mine, to clarify how such maladaptations can influence conditions.

The patient is 60 yr old male, with pain on his neck for about five months prior to our first appointmentSome 20 years ago he had a fusion of his C3-5 due to what he recalls as a disc prolapse. He had actually been reasonably good since, until gradual and significant increase of his pain some five months ago.
X-ray showed significant arthritis and degeneration on the vertebrae above and below the fusion site. This is reasonably common for those who had fusion. There were also signs of marked spinal cord and nerve root compression.
He has already been to a specialist, who recommended against any further surgery.
At the time of our initial session, he had been quite afraid of moving the neck for a number of months, due to his fear of permanent paralysis. He literally believed that every time he moved his neck into pain, he would be hitting onto his spinal cord and nerves and inching his way into a permanent nerve damage.
(Which is, from a patient’s perspective, fair enough. But paralysis and nerve damages simply don’t happen like that.)

So I explained:


His arthritis would have happened very gradually, let’s say over the course of at least ten years for argument’s sake. His pain really has been occurring for five months. The difference of arthritis between six months ago when he didn’t have pain, and five months ago when he did start having pain, would have been minimal. So what changed?

We could rationalize that there was a certain “tipping point” where the arthritis begins to be truly symptomatic. And I think that that's a valid explanation and this "tipping point" could have happened for this patient when he began to have a lot of pain some months ago.

However the presentation that I was seeing on the patient was not so much pain as a result of degeneration or arthritis, but it was consistent with his pre-emptive fear of worsening of his condition. We call this hypervigilence (3,4). How did I judge that it was more to do with hypervigilence rather than actual joint arthritis? I arrived on that conclusion only after a very thorough assessment and clinical reasoning.

So I rationalized that his condition was more to do with the fact he was hypervigilent and that was actually increasing stiffness and pain. He was pre-empting pain and moving like “this” when he could actually move upto here and here quite comfortably. He was operating within his self-imposed limitation, that was unnecessarily restrictive.

Not only is that self-imposed limitation unnecessarily restrictive, it would have actually resulted in maladaptation response from his body that results in pain. But THIS pain would be from maladaptation and NOT from the primary problem which was arthritis and degeneration.

Unfortunately patients cannot distinguish between these two and they think that the pain from the maladaptiation is from the arthritis. And this perspective simply but unfortunately, reinforces the original viewpoint that pain is worsening BECAUSE of arthritis, whereas in truth the pain might have been TRIGGERED due to arthritis but is PERSISTING or worsening because of his own hypervigilence.

It’s a vicious cycle in action: it’s sore, he doesn’t move because he’s afraid, it becomes sore because he doesn’t move, and he moves less because of worsening pain.

So I told him that while the state of his bones was likely to be responsible for some amount of discomfort, his presentation was consistent with maladaptation rather than arthritis. So I asked him to gently move his neck and try to function as normally as possible, like what he had been doing just prior to the onset of his neck pain five months ago.

I also assured him that his joints were deemed stable by not only myself but the surgeons as well, and any chance of permanent neurological damage such as paralysis was low, and his body is perfectly capable of creating a positive adaptation to minimize the impact of his degeneration.

And I did some manual therapy, with a purpose of demonstrating to him what his neck was really capable of and thankfully he really agreed with everything I had to say.

And he improved well. Really well. And most important of all, he gained peace within himself. This is the key to any rehabilitation, but especially something like chronic or complex pain.

We see these cases all the time. Where patients make less-than-ideal judgment on his or her own condition and the patient's experience of pain is more to do with maladaptation response that arise from consequences of such judgment, rather than actual primary condition.

So, the key messages are:

1) The limitations that you put on yourself sometimes does more harm than good

2) That’s because of any shift that you have on your cognitive space, positive of negative, will result in behavioural changes, which results in concordant physical adaptations.

3) How you use YOUR body’s ability to adapt, positively or negatively, is up to you

4) Inner peace is important. It's a prerequisite for any successful rehabilitation and both patients and clinicians must strive to achieve it.

Injury rehabilitation and pain management - three critical principles.

First of all, for sake of clarity, let’s differentiate three scenarios where you can experience pain:

1. You are in pain due to some structural compromise – like fractures, joint sprains and so on

2. You are in pain, but do NOT have any structural compromise – like headache, repetitive strain injuries, fibromyalgia, or irritable bowel syndrome

3. There is some level of structural compromise but how much it contributes to pain is unclear – arthritis is a very important example of this, and so is chronic back and neck pain

Regardless of on which scenario or scenarios your pain might fall, or how simple or complicated it might be, injury rehabilitation can be explained in very simple principles. But behind these simple principles, the science is very complex and still not very well understood. Today, we’ll focus on the simple principles.

The three principles of injury rehabilitation or pain management are:

1. Settle the acuteness
2. Let structures heal (only if there’s a structural compromise)
3. Get on with your life

First, settling acuteness means settling two physiological processes that are crucial but if unnecessarily sustained, can prolong pain. These are:

- Inflammation and
- Sensitisaiton

Inflammation is a chemical process that naturally occurs if something is compromised, and generally shows up as swelling (although not always). Sensitisation is a process where the nerves become extra-sensitive to pain(1). Like inflammation, it’s a normal process, and a good example is being sore to touch on the area where you've hurt yourself.

Method of settling down these processes can vary greatly, depending on your condition. Settling down a wrist fracture is going to be vastly different from settling down a carpal tunnel syndrome. Sometimes, you will have to rely on a competent health professional to guide your way.

Once the acuteness is settled, then we must consider the second principle - letting any compromised structures heal. But with this, comes a big disclaimer:

This principle ONLY applies when there actually IS a structural compromise that is RELEVANT to your condition.

Sounds quite obvious doesn’t it? But trust me, I've seen countless people WAITING for something to heal up when there was no relevant structural compromise to begin with.

(People often hang around here far too long before moving onto the next principle).

And while all three principles are important, this third one is the most pertinent point for patients because this is the only principle that is absolutely and entirely up to you. And this is also by far the most underestimated aspect of recovery. What I’m talking about is the third and final principle, "getting on with your life".

There have been countless researches investigating FACTORS that turn an injury into persisting pain, or continuation of persisting pain that disturbs quality of life (2,3). And it’s not the severity of injuries, it’s not age, and it’s not even degeneration or arthritis. It’s fear(4).

It's fear of pain that stabs you when you lift something or turn your head. It’s fear of not recovering from pain if you return to work. It’s fear of returning to normal life because the pain would get worse.

However it’s the very fear of pain getting worse if you returned to your life, that is actually perpetuating the pain, resulting in poor outcome of injury rehabilitation. And I’m not talking about “pain all being in head” or “imaginary pain” sort of thing. Or often-used but very incorrect (in my opinion anyway)"somatoform" or "non-organic" pain. It is actually a real physiological process.

When a person experiences an injury of pain, there are adaptive changes that occur at neural level. That is how your brain and central nervous system controls the body. From muscles to hormones, from movement to respiratory. These changes are ok or even beneficial in the early stages of injury rehabilitation, like protecting a fracture, but these adaptive changes do NOT help you at all once the fracture is healed, and they are downright destructive if there were NO relevant structural compromises to begin with.

They’re MAL-ADAPTIVE, at celluluar, neural, cogtive, behavioural, and in every facet of your body. Much of the rehabilitation that we do for fibromyalgia or chronic pain condition is to identify and untrain these maladaptative changes.

So the point that I want to get across is, getting on with what you’d consider “normal” life is incredibly important if you’re working on improving your condition.

(Unless your normal is doing "nothing"; then clearly you need to recalibrate what is normal).

Now I’m in a great danger of sounding like someone who tells a person struggling with depression, to “just get over it”. That’s not my intention at all. However, in a way, if you’re having chronic pain, it’s actually NOT unlike recovering from depression. And just like managing depression, it’s very helpful or sometimes necessary, to have a competent health professional who will guide you, encourage you, sometimes challenge you, and help you to be resilient to adversity.

And this “getting on with your life” or “returning to normal life” can actually begin VERY early. If possible, you can go to work with your broken bones, you can still go to your gym with migraines, you can still do housework with back pain. It might be difficult at times, but in a long-run you’ll be much better off.

Again, I stress, it all depends on YOUR condition. That’s why a good health professional is so important when it comes to injury rehabilitation or pain management. But just remember, maladaptive changes need to be eliminated as soon as possible. How that can be done, whether you use a brute will or gentle pacing or somewhere between, that’s the important question.

Let me summarise again. Three principles of injury rehabilitation or pain management, unusually, but not necessarily in order:

1. Settle the acuteness
2. Let structures heal (only if there’s a relevant structural compromise)
3. Get on with your life

Types of pain – Clinical application

Knowing what type or types of pain someone has is not much use if you don't know how to use the knowledge and clinically reason your way to an effective management. In today's video I'll attempt to briefly explain what it all means.

It's necessary to know the types (or more accurately "mechanism") of pain, simply because each type of pain has different physiology that is driving the pain, and thus requires different interventions or INTENTION behind interventions. But first, we need to recognise different mechanisms of pain when we see one.

For example:

- a 1-day-old ankle sprain is probably full of inflammation, whereas pain four months after a sprain is more likely to be dysfunctional.

- a chronic neck pain may be to do with central sensitisation which is dysfunctional, or muscle insufficiency which may be more nociceptive.

- a carpal tunnel syndrome may be quite neuropathic and inflammatory, or it may be more dysfunctional.

- a back pain could be nociceptive due to the pain straight from a disc that's been stressed beyond its adaptive capabilities , it may have a neuropathic component if it has an acute nerve root compression, and if chronic it may be dysfunctional due to central sensitisation. There may even be some inflammation around the area as well.

And of course, in most cases it's a mixture of the these four mechanisms of pain. It's simply a matter of recognition and prioritising.

Then how do we recognise the different mechanisms of pain of a given person? That is a pretty big question, and while there are some typical presentations or patterns for each mechanism of pain I just can't go into it in this video.

Same with proposed interventions for pain. It's a big topic with complex and conflicting evidence. We must also acknowledge that perspectives, working paradigms and biases vary greatly across health disciplines AND within each discipline as well, so a same patient can receive quite different treatments from different clinicians and it's very difficult to say who is right and wrong.

However what I can say is that it would be wrong NOT to consider these four mechanisms when caring for patients with pain. At least an attempt at differentiating them will give us much clearer picture of management pathway.

And then we must decide whether the particular mechanism or mechanisms of pain actually needs active management (since most pain self-resolves), and if it does need management how we, the clinicians can positively influence it, with firm consideration of multidimensional nature of pain (1,2).

Types of pain – Dysfunctional pain

It is generally accepted in the branch of pain medicine that there are four types of pain: nociceptive, inflammatory, neuropathic and dysfunctional (1-3). The first three have been discussed in previous videos and in this video we'll discuss about dysfunctional.

Dysfunctional pain is tricky to define AND to understand, some will even say that it does not exist. Regardless, let's start with the definition of dysfunctional pain: dysfunctional pain is pain experienced when there is an error in pain processing ability of the body.

This means that there's likely to be NOTHING that's structurally wrong with the patient; X-ray will be normal, MRI will be normal, blood test will be normal, and your muscles and joints and fine. And you might be thinking “hang on a sec, how can you have pain without something wrong with me?"

Well I'm not saying that there is NOTHING wrong with you; it's just that where it's gone wrong is not something that can be SEEN. And THAT is the pain processing centres of the body.

I'll give you an example. Let's say that you've just cut yourself on your finger, it hurts right? I'm sure you can remember that searing pain that is when you've just cut yourself. And that's perfectly normal to feel pain. Now, how long does it take for that initial pain to settle? Maybe ten minutes? Thirty if it's particularly bad.

So when the pain has settled after ten minutes or so, does that mean that your cut has been healed? Not at all, it might take at least a day to really heal up.

So, why does your pain reduce so significantly so quickly, unrelated to actual healing? There are a number of reasons for this but one of the major ones is our pain processing centres. They go - "oh look, a cut on my finger. It's just a cut. I'm sure it'll be fine". Then turn down the initial spike of pain pretty quickly. And this all happens without you being quite aware.

But what if that "turning down the pain" by the pain processing centres does NOT occur? The pain will persist, quite unreasonably, even well after the cut is healed. That's dysfunctional pain. Pain that persists because of errors in our pain processing ability.

Sometimes this occurs even without a particular injury per se. In fact it's most common that this unreasonable persistence of pain occurs from gradual build up rather than one-off trauma. Think chronic back and neck pain, shoulder pain, tennis elbow, most overuse injuries, headaches, irritable bowel syndrome, fibromyalgia, the tension that you feel when you're anxious or stressed, the list goes on.

For now, the most crucial thing for you to understand is the very existence of dysfunctional pain, how it can manifest without any visible structural abnormalities, and how common it actually is.

In summary, there are four types of pain: nociceptive, inflammatory, neuropathic and dysfunctional. What does that mean in real life and how do we apply this concept? That is a pretty big question, and I'll try to explain it some degree in the next video.

Managing chronic pain is a bit like scaling a mountain. Except you have no idea how high the mountain is.

Discouragement is a part of the journey, thus handling such emotion is one of the keys to managing pain. A competent and passionate health professional should be able to be your guide through these peaks and troughs.

Sean* tells us his journey and perspective. I think he has some great points that anyone going through chronic pain can relate to.

Types of pain – Neuropathic pain

The third type of pain that I want to talk about is Neuropathic pain.

Neuropathic pain occurs when you have pain because there is some sort of a damage or stress to our nerves (1). Now this can sound awfully like nociceptive pain that we discussed two videos earlier, where certain nerves called nociceptors get simulated and send signals to our brain that can be interpreted as pain.

The difference is, in nociceptive pain, the nociceptors are responding to stresses to SURROUNDING structures, like ligaments, muscles, or bones. Neuropathic pain occurs when the nerves THEMSELVES are compromised, and begin to behave rather irritably.

For example, in shingles, a virus attacks the nerves in our spinal cord, and consequently you'll have pain, and even blisters, wherever that nerve goes. Some with diabetes develop peripheral neuropathy where the nerves in the hands and feet become compromised.

And one of the most common neuropathic condition is "sciatica" where the nerves that come out of the spine to supply one of the legs become compressed by something - usually a bulging disc.

It is also probably worthwhile to mention that when this does happen there are effectively TWO conditions that we're dealing with - a bulging, unstable disc, AND an angry, irritated nerve.

That means even if you have a surgery to slice off the part of the disc that's pushing on the nerve, the irritated nerve does not always resolve automatically and THAT may require some separate attention. Something like carpal tunnel syndrome is similar in that regard.

So these are the most critical aspect of neuropathic pain:

1) neuropathic pain is a separate entity to the condition itself. The condition, like shingles, may have passed, but the pain might remain. It's not like where you break a bone and the bone's fixed and you'll be ok. The PAIN needs to be looked at separately (2).

2) the behaviour of neuropathic pain is not as predictable as the previous two types of pain. In nociceptive and inflammatory pain, the resolution of the pain is pretty consistent with a number of factors. Neuropathic pain, often is not so.

3) and lastly, and this is more for the health professionals, neuropathic pain needs to be intervened quickly.

So if you see someone with a suspected bulging disc and raging sciatic pain, don't just settle for finding a solution only for the disc, you must treat the neuropathic pain separately, and AS EARLY AS POSSIBLY, before alterations in neuroplasticity and in gene expressions occur both of which will prolong the pain very much unnecessarily (3). And this multimodal and mechanism-specific approach applies to most neuropathic pain conditions.

Types of pain – Inflammatory pain

The second type of pain that I would like to talk about is, pain due to inflammation.

Inflammation is a chemical reaction, that usually occurs when there is a damage in our body. Let's say, an ankle sprain. Initially you'll feel the nociceptive pain of mechanical origin, of the ankle being twisted or ligaments being torn.

Then, if the injury is great enough, inflammation will occur. It's a chemical reaction where certain cells in our body release what we call inflammatory mediators.
And these mediators cause two major consequences:

First, it causes the blood vessels to increase in size, and the plasma in the blood goes out of the blood vessels and into the tissue space, which is essentially space between our tissue cells like muscles and fat. The result is swelling.

Secondly, the inflammatory mediators stimulate the nociceptors around the injury site. So you'll end up with that constant, throbbing, achy pain that accompanies most moderate to high level sprains or injuries.

However, this sort of inflammation does not last long. Maybe three to four days in most musculoskeletal injuries. It literally sorts itself out. Right amount of inflammation is helpful to healing as well.

But sometimes, things can go south. I'm not sure if you can have too little an inflammation, but you can definitely have too much. Also, you may have unnecessarily excessive response from your nociceptors resulting in unreasonable amount of pain.
You could also have chronic inflammation, which is quite different again.

The take home message is, acute inflammation in the realms of musculoskeletal pain (so, not like appendicities), usually follows a decent trauma and is self-limiting.
This means that a lot of chronic pain, like fibromyalgia or chronic back or neck pain, has not much to do with acute inflammation and will not respond well to anti-inflammatory drugs.

Of course there are so much more to this, like systemic inflammation, chronic inflammation, and visceral inflammation and they are all quite different. It is really upto the critical eyes of a competent health provider of your choice who can say how much portion of your pain is due to inflammation, and what can be done about it.
